Stop Writing One Paragraph Per Study: How to Synthesize a Literature Review

To synthesize a literature review, organize the evidence around ideas, patterns, disagreements and gaps rather than around individual authors. Compare studies within the same paragraph, explain why findings converge or differ and end each section with what the body of evidence collectively means. A synthesis matrix can help you move from “Study A found…” to an argument supported by several studies.

You have not necessarily done much synthesis.

A literature review is not supposed to be a queue in which every paper waits for its own paragraph.

What does “synthesize the literature” actually mean?

Synthesis means bringing evidence from multiple studies together to say something about the body of evidence.

The shift is simple to describe:

Stop asking:

What did each study say?

Start asking:

What do these studies, considered together, tell us?

That means identifying:

  • recurring findings;
  • areas of disagreement;
  • differences between populations or settings;
  • methodological reasons results may differ;
  • patterns across time;
  • weaknesses in how the evidence has been produced;
  • questions the existing evidence still cannot answer.

Your citations support the argument.

They should not become the structure of the argument.

Read one paragraph of your literature review.

If the paragraph is organized like this:

Author A found X. Author B found Y. Author C found Z.

you are probably summarizing studies one at a time.

If it is organized like this:

Across several settings, X appears consistently important, although the mechanism differs between A and B. Studies using C tend to report..., whereas studies in D show...

you are beginning to synthesize.

Bench rule

The paragraph should have an idea even after you hide the citations.

If removing the author names leaves no argument behind, there is probably more synthesis to do.

Start with a synthesis matrix

Do not try to synthesize 30 studies entirely from memory.

Build a matrix while reading.

Suppose you are reviewing barriers to contraceptive use among adolescents.

Useful columns might include:

After several papers, stop reading down the Study column.

Read across the themes.

You may see:

  • confidentiality;
  • fear of side effects;
  • provider judgement;
  • partner or parental influence;
  • cost;
  • distance;
  • service availability.

Those concepts can become the structure of the review.

Not the surnames.

Weak version

Namusoke et al. found that confidentiality was a barrier to adolescent service use. Okello et al. also found that confidentiality was a barrier. Smith et al. found that young people feared being seen at the clinic.

All three studies have been reported.

But the reader still has to perform the analysis.

Stronger version

Concerns about privacy appear consistently across studies of adolescent service use, although the mechanism differs by setting. Facility-based studies often emphasize fear of disclosure during provider interactions, while other studies show that simply being seen entering a sexual and reproductive health service can expose young people to social judgement from parents or peers. Together, these findings suggest that confidentiality extends beyond what happens inside the consultation room to the social visibility of seeking care.

Now the paragraph has an argument:

Confidentiality is broader than provider secrecy.

The studies support that argument.

That is synthesis.

How do I organize a literature review by themes without becoming superficial?

A thematic structure is not automatically a synthesis.

You can still create a section called “Confidentiality” and then write six study summaries underneath it.

Within each theme, ask:

What is consistent?

Do multiple studies point in the same direction?

What differs?

Are the findings different across groups, settings or methods?

Why might they differ?

Could design, measurement, service context, age, geography or sampling explain the variation?

How strong is the evidence?

Are the studies mostly small qualitative studies? Large probability surveys? Cross-sectional analyses? Intervention studies?

What does the pattern mean?

What can you conclude cautiously from the evidence as a whole?

That final question is usually where synthesis begins.

What should I do when studies disagree?

Do not write:

Some studies found an association while others did not.

That is accurate and almost useless.

Ask why.

For example, suppose several studies report that distance is an important barrier to accessing care while others find little relationship.

Look at:

  • rural versus urban settings;
  • actual travel time versus straight-line distance;
  • transport cost;
  • whether the service is available once the person arrives;
  • differences in age or mobility;
  • whether the study measured perceived access or geographic access;
  • study power and precision.

Disagreement is not a nuisance to be hidden.

Sometimes heterogeneity is the most interesting finding.

How do I move from synthesis to a research gap?

A good gap should emerge from what the evidence fails to establish.

Imagine your synthesis shows:

  1. Provider attitudes consistently affect adolescents' experiences of care.
  2. Studies disagree about whether privacy problems arise mainly from providers or from being seen using services.
  3. Most studies describe the problem, but few examine which changes to service organization reduce these concerns.

Now the gap is not:

No study has been conducted in District X.

It is:

Existing research identifies confidentiality and provider trust as important, but provides less evidence on which service-level changes can reduce the social and interpersonal risks adolescents associate with seeking care.

That is a much stronger reason for doing a study.

What do I actually write?

A useful synthesis paragraph often moves through five steps:

Claim → evidence across studies → comparison/contrast → explanation → implication

For example:

Across settings, provider attitude emerges as a recurring influence on adolescents' willingness to use sexual and reproductive health services. However, the form of this concern varies. Some studies emphasize overt judgement during provider interactions, while others highlight fear that providers will disclose attendance or that adolescents will be recognised when entering services. These differences may reflect variation in service organization rather than disagreement about the broader role of trust. Existing evidence therefore establishes the importance of the provider encounter but provides less clarity on which service changes most effectively improve perceived confidentiality.

Do not treat that as a paragraph template to fill mechanically.

The point is the reasoning sequence.

What about systematic reviews?

Formal systematic reviews require much more than writing good synthesis paragraphs.

Methods such as meta-analysis, synthesis without meta-analysis and qualitative evidence synthesis have explicit procedures for grouping, analysing and reporting evidence.

If you are writing a conventional thesis literature review, you do not need to pretend you conducted a Cochrane review.

But you can borrow an important discipline from evidence synthesis:

Group deliberately. Compare explicitly. Make your reasoning visible.

Do this now

Take the last three paragraphs of your literature review.

Highlight every sentence that begins with:

  • “Author et al...”
  • “A study by...”
  • “According to...”
  • “Similarly...”

Choose one paragraph and rewrite its first sentence so that it begins with the finding across studies, not an author's name.

Then use the studies as evidence.

Frequently asked questions

How many studies should I include in one literature-review paragraph?

There is no fixed number. Include the studies needed to support, compare and qualify the paragraph's analytical point. Three well-integrated studies can be stronger than eight citations listed without interpretation.

Can I organize my literature review by themes?

Yes, if the themes help answer your review question and you synthesize evidence within them. A thematic heading does not automatically create synthesis.

Should every study I read appear in the literature review?

No. Reading is broader than citation. Include studies that materially contribute to the argument, context, comparison or methodological assessment.

How do I show disagreement between studies?

State the pattern of disagreement and then examine plausible reasons for it, including differences in population, context, methods, measurement and precision.

Is synthesis the same as meta-analysis?

No. Meta-analysis is a specific statistical form of quantitative synthesis. Narrative and qualitative forms of synthesis also bring evidence together, but use different methods.
